Resilience Behavioral Health is seeking a Case Manager/Group Facilitator for their outpatient substance use and mental health treatment program in Willowbrook, IL. The program provides PHP, IOP, and OP services to adults (18+).

We’re looking for a Case Manager/Group Facilitator who’s ready to jump in and inspire change. In this role, you'll lead group discussions, meetings, and workshops that empower clients to grow, connect, and achieve their goals. You'll also deliver hands-on case management services — creating personalized treatment plans, coordinating continuing care, and supporting smooth discharges.

You'll work closely with our dynamic Treatment Team, maintain strong relationships with referral sources, and help clients access the resources they need for long-term success.

CASE MANAGER/GROUP FACILITATOR PAY: $25/Hr. 

CASE MANAGER/GROUP FACILITATOR DUTIES AND RESPONSIBILITIES:

 
  • Facilitate group discussions, meetings, and workshops to encourage collaboration and achieve goals.

  • Provide individual case management sessions to support client needs and recovery goals.

  • Document client progress accurately and timely in the Electronic Health Record (EHR).

  • Communicate psychosocial and diagnostic information clearly to the treatment team.

  • Coordinate with the treatment team to ensure comprehensive, individualized care planning.

  • Assist clients in accessing community resources and managing external stressors.

  • Support clients with continuing care and discharge planning.

  • Maintain open communication with families and provide them with support and resources as appropriate.

  
CASE MANGER QUALIFICATIONS: 
  • Bachelors degree required in either social work, counseling, human services, business administration, or closely related field.
  • One year of experience in a substance use and/or mental health treatment environment required.
  • Experience in start-up environments is preferred.
  • Demonstrates critical thinking, sound reasoning and judgement.
  • Effective communication skills and processes.
  • Fosters a healthy and positive team-approach with staff.
  • Demonstrated computer proficiency.
